WHAT TO INCLUDE WITH YOUR REQUEST:

  1. Your Request Form
  2. Signature Authorization from the Employee

Request must include the Employee's name and another verifiable piece of information - e.g. Employee's DOB, last four of SSN, current address, etc.

RESPONSE TURNAROUND TIMEFRAME:

  • 5 - 7 Business Days
