Teacher and Support Staff COVID-19 Hazard Pay

Dear Teacher Staff:

The Michigan Department of Treasury has issued the Teacher COVID-19 Grant. It has been determined you may be eligible to receive this grant. Included in this email is the link to the electronic Form 5734. If you do not know your PIC number, you can find that information on our website along with the FAQs. The FAQs document from the State of Michigan is located at the following link Grant Eligibility FAQ.

The COVID-19 Grant eligibility criteria was established by the State of Michigan. Southfield Public Schools (the "district") did not provide input on the criteria nor does it have the ability or the authority to modify the criteria established and outlined below. Please read this email in its entirety to see if you are eligible.

In order to receive the grant, Teachers will need to meet the following eligibility requirements:

  1. Prior to the issuance of executive order 2020-35 on April 2, 2020, the teacher performed at least 75% of their standard instructional workload in a brick and mortar classroom in a District.
  2. After issuance of executive order 2020-35 on April 2, 2020, the teacher developed tools and methods to deliver distance learning, take-home packets, or other methods described in the District's continuity of learning plan.
  3. The teacher certifies to the District that he or she worked additional time spent outside of normal working hours, experienced hazardous conditions, or incurred additional costs related to ensuring students could effectively participate in the District's continuity of learning plan. (This form will be reviewed by the district for eligibility. If the teacher was employed in a part-time capacity, the $500.00 hazard pay will be prorated to meet their portion fulfilled.)

What does an individual need to complete in order to receive the grant money?

  1. Are you eligible?  Read the Grant Eligibility FAQ and eligibility requirements.
  2. Review Form 5734 to see if you meet the eligibility requirements.  If you do, then completely fill out Form 5734 located at this link, electronically sign it and send to the following email address spsform5734@southfieldk1org.
  3. Make sure that Southfield Public Schools has your current address by logging into your Employee online by December 4, 2020. You can update your address through this website.

Timeline:

  • November 9, 2020 - The district sent notification to eligible staff and provided them a link to an electronic certification form.
  • December 4, 2020 - Electronic certification form is due back via email to spsform5734@southfieldk12.org. The form must be received by the District by December 4, 2020. Incomplete forms and forms received by the District after December 4, 2020 will not be processed.
  • November 9, 2020 - December 4, 2020 - The time period an individual can appeal the ineligible status.
  • December 16, 2020 - The District submits the information to the State of Michigan in order to process the grants.
  • February 25, 2021 - The Michigan Department of Treasury estimates mailing checks on or about February 25, 2021, to the address of residency provided via the electronic certification form.

Upon reviewing each submitted electronic certification form, if the district determines that you do not meet the eligibility requirements released by the Michigan Department of Treasury you will be notified via email. If you believe the District’s determination that you are ineligible for the grants is an error, you may submit your written appeal for further review to spsform5734@southfieldk12.org.

Appeal Process
All appeals must be received by December 4, 2020. If you did not receive an email and believe you are eligible for the Teacher Support Grants, please send an appeal to the email spsform5734@southfieldk12.org. In that appeal, please indicate the rationale for your appeal and include a telephone number in which to contact you.  

State of Michigan Audit Process
The Michigan Department of Treasury can audit any Form 5734 received by the District. Individuals who submit Form 5734 to the District, may have to provide documentation in order to verify their statements that were submitted on Form 5734 are accurate.

For a list of school support staff positions please refer to the paraprofessional/aide assignments beginning at p. 41 and non-instructional assignment codes beginning at page 43-46 of the Registry of Educational Personnel Data Field Descriptions Manual. The State of Michigan has also compiled a Grant Eligibility FAQ.
  3. The school support staff certifies to the District that he or she worked additional time spent outside of normal working hours, experienced hazardous conditions, or incurred additional costs related to providing student services during the COVID19 pandemic. (This form will be reviewed by the district for eligibility. If the support staff was employed in a part-time capacity, the $250.00 hazard pay will be prorated to meet their portion fulfilled.)

Per the information received from the Michigan Department of Treasury individuals who worked on behalf of the District as Non Instructional School support may be eligible for the School Support Staff grant in an amount up to $250.00.

In order to receive the grant, Non Instructional School support staff will need to meet the following eligibility requirements:

  1. Prior to the issuance of executive order No. 2020-35 on April 2, 2020, the school support staff (a K-12 full-time or part-time paraprofessional, aide, or non-instructional staff) performed at least 75% of their workload in a brick and mortar school building in a district. For bus drivers and bus aides, the statement of brick and mortar is not required, but the individual needs to work in the District prior to April 2, 2020. The individuals had to continue to work through June 30, 2020.
  2. Meets the school support staff statutory requirements to receive grant funding. For a list of school support staff positions please refer to the paraprofessional/aide assignments beginning at p. 41 and non-instructional assignment codes beginning at page 43-46 of the Registry of Educational Personnel Data Field Descriptions Manual. The State of Michigan has also compiled a Support Staff Eligibility FAQ.
  3. The school support staff certifies to the District that he or she worked additional time spent outside of normal working hours, experienced hazardous conditions, or incurred additional costs related to providing student services during the COVID19 pandemic. This form will be reviewed by the district for eligibility. If the support staff was employed in a part-time capacity, the hazard pay will be prorated to meet their portion fulfilled.

If you are not able to email the document, then you can call (248) 746-8500 to make an appointment to pick up and/or drop off your application. You must pick up your document no later than November 23, 2020.
