Procedure
- Employees injured at work should complete the Employee Injury Report (attached below) and forward it to their Manager/Principal and the Chief Human Resources Officer.

Retirement
So You Think You're Ready To Retire?
- Contact the Office of Retirement Services (ORS) for eligibility and confirmation of retirement.
- Email SPS Human Resources expressing your intent to retire.
- Submit the SPS Retirement/Resignation Form to SPS Human Resources.

Once HR receives your form and confirmation from ORS, your retirement request will be processed completely with the district, on the retirement effective date that you select.
